Worksheet doesn't give the option for student to enter name and send to teacher
Worksheet doesn't give the option for student to enter name and send to teacher. I have followed the instructions for "custom links" but when completed, the student clicks "finish" and is asked to enter a name, but there is no place to do that.

To solve this issue. You can edit the worksheet, please note the Submit Mode needs to be default. Please refer to the screenshot in the following link:
