Dear colleague, I like your test very much but may I suggest you make some corrections to this test? In the 6th sentence the main verb is DO, and you put MAKE in the right answer. In the sentence no. 10 your correct answer should have NEW MITTENS because that is what is in the active sentence. And in the last sentence the correct answer should be "by them" not "by us", as you wrote.I hope you will make the corrections because I already gave it to my students to do it online and we love it. Best regards. :)

I agree with all JelenaK's corrections, I think the same. The test is lovely, but I have some more corrections to suggest.
For example, in the sentence "Sally wakes her brother up every morning." for me it would be logical to transform it into "SALLY'S BROTHER is woken up BY HER/BY SALLY every morning". And the same story with Bill's bike, for me it would be logical to change it into "BILL'S BIKE was fixed BY HIM/BY BILL yesterday afternoon.", because in your variant I understand it as Bill is fixing bike for somebody else, and it changes the original active sentense's sense.Thank you very much.
Best wishes!
